Saskatchewan Organ and Tissue Donor Registry

How to register

Registering your decision for organ and/or tissue donation is easy – all you need is your Saskatchewan Health Services Card number, first and last name and date of birth.

You can choose to donate:

  • all organs and tissues needed for transplant; or
  • select organs and tissues of your choosing.

All options are outlined in the registry.

Register your decision in the following ways:

  • Online from any computer, tablet or mobile device; or
  • By paper form.

Once complete, the paper forms need to be sent to:

They can also be scanned and sent by email to donorregistrysupport@ehealthsask.ca. If you register by paper form, your donation choices will be entered into the registry on your behalf.
Once you have registered your decision, you can revisit this registry at any time to confirm or modify your choices.

After you register, be sure to talk with your family (or whoever would be considered your next-of-kin), so they understand your decision. Families are more likely to agree to donation if they are aware of a family member's wishes.
