Immunization Records

For immunization records, please contact your local Public Health office or First Nations Community. The public health staff will need information including your date of birth, health card number, previous address, and school(s) attended. There may be an administration fee for records provided.

MySaskHealthRecord

For quick and easy access to yours or your family’s personal health information, including immunization history as recorded in Panorama (the provincial immunization registry), register for MySaskHealthRecord.  

CANImmunize

CANImmunize is a free app that provides you with the ability to:
  • Easily record and save your family's immunization records and information;
  • Access vaccination schedules; and
  • Manage vaccination appointments for the entire family.

It is important to know that MySaskHealthRecord and the CANImmunize app are not linked together.

Any immunization  information self-recorded in CAN Immunize is not considered an official record.
