Proof of COVID Vaccination

COVID Vaccination Record Available with QR Code

Saskatchewan residents with a MySaskHealthRecord account at eHealth Saskatchewan are able to view their COVID-19 vaccination record in a format that includes a QR (Quick Response) code. Information is available about adding your immunization information to your Saskatchewan health record if you have been vaccinated outside of the province, or if your immunization information is missing. Information is available for out of province residents (including Lloydminster) who received vaccination in Saskatchewan.

It can be downloaded/saved, printed and viewed on mobile devices.

This format with QR code includes the individual's name, date of vaccination, country of issuance, type of vaccine administered and lot number.

The certificate with digital QR code supports proof of vaccination at border entry where required when travelling internationally, and is aligned with Canadian federal government policies.

Your QR code is static and must be updated if your vaccination status changes, such as after receiving a booster dose. Access your account and generate a new QR code after receiving any COVID-19 vaccine, to ensure your record is up-to-date.

You will have several options for showing your proof of vaccination credentials;
  • QR Code on your mobile phone (screenshot from MySaskHealthRecord or from MySaskHealthRecord mobile app)
  • QR code printed from MySaskHealthRecord
  • COVID-19 vaccine certificate printed from MySaskHealthRecord
  • Wallet cards received at time of immunization (Saskatchewan Health Authority or Indigenous Services Canada)
  • Proof (electronic or in writing) issued by another province, territory or government.

Please note: The above are acceptable proof in Saskatchewan, but may not be acceptable in other jurisdictions.

To register for MySaskHealthRecord, key identity verification documents (health card and SGI photo ID) are required. Once an account is created, individuals may access their COVID-19 vaccination record as well as their complete immunization history, lab test results, including COVID-19 tests, and other health history. It may take up to 24 hours for your record to be updated, after your account is activated.

Affidavit Submissions

Some individuals chose to interpret the public health measures as a mandatory immunization policy, and have secured affidavits objecting to immunization. As immunization is not mandatory, exemption is not required and these affidavits serve no purpose.

The Saskatchewan Health Authority requests that individuals do not submit an affidavit in response to COVID-19 immunization. Affidavits are not entered into any health database and will be destroyed on receipt.
